COMMUNITY BASED PALLIATIVE CARE is an approach that improves the quality of life of patients and their families facing the problem associated with life-threatening illness, through the prevention and relief of suffering by means of early identification and impeccable assessment and treatment of pain and other problems, physical, psychosocial and spiritual

Objectives

•    To empower the local community to look after the chronically ill, elderly and bed-ridden people in their area

•    To establish a cost-effective, and locally driven, system for provision of palliative care in the community

Strategy

To encourage local people to address the social needs of the patients and families, train the community volunteers to offer emotional support, facilitate the development of locally sustainable home care programs and to establish a network of nurses and doctors with expertise in palliative care to support these initiatives. It is being attempted to supplement the efforts of trained doctors and nurses in psychosocial and spiritual support by trained volunteers in the community. Groups of trained volunteers are tied to Palliative Care professionals and health care facilities in their communities. Community Volunteers are responsible for setting up palliative care units in the network

•    Initiate and run palliative care units in their locality, Visit patients at home (both with the home care unit and on their own),Help at the outpatient clinic (Keeping the patients comfortable, talking to them, helping with clerking etc),Administrative work (including clerical work and account keeping)

•    Raise funds for the unit and Mobilize support for the patients from the various governmental and nongovernmental agencies. Funds for the activities of the units are generated through donations from individuals and institutions

TAMIL NADU INSTITUTE OF PALLIATIVE MEDICINE(TNIPM)

The TNIPM acts as a Nodal centre to promote and demonstrate community based models, though action research and training institution of Pain and Palliative care Initiatives in Tamil Nadu, it is also influencing policies in age care and palliative medicine. It was constructed in Cuddalore City to make palliative care service available & accessible to all those in need of it. It consists of an

• Home care / out /In-Patient Facility

Where patients are given treatment at the Home care service if the patient are not able to move from home. Weekly one there will be OP clinic at Cuddalore Pain Palliative Cline at our office. If they require inpatient service for less period the patient were admitted to bring difficult symptoms under control or to provide respite care

• Training institute / School of Nursing

Palliative care is created & doctors, paramedical staff; volunteers and family members trained to care for patients better.

• Research and documentation

All the best practices would be documented for other to follow, and every action there will be baseline and will do the impact assessment. In this we have taken a documentary fill and done impact study on community based palliative care service. In implementing any project we the institute would undertake the action based research and implementation
